Allure Academy is neither involved in the review nor decision-making process of any criminal history evaluations conducted by the Texas Department of Licensing and Regulation. The Academy is not responsible for any adverse outcomes. Please note that having a criminal history does not automatically exclude an individual from obtaining a license.

Every individual who applies for a license with the Texas Department of Licensing and Regulation (“the Department”) is subject to a criminal background check to determine his or her suitability for the license. If an applicant knowingly has a criminal background, the applicant is highly encouraged to contact the Department before applying to determine if it will hinder their ability to become licensed in the state. Section 51.4012 and Chapter 53, Subchapter D, of the Occupations Code allow a person to request a criminal history evaluation letter from the Department before applying for a license.

Before you submit the application, please note:

1.If you have questions during the application process, be sure to contact us (contact information is included below on this form) to discuss all of your queries. We are dedicated to helping you pursue your educational dreams.

During the application process:

1. Answer all questions in this form as accurately as possible.

2. Once you complete your application, you will need to submit a non-refundable application fee of $25. This non-refundable processing fee is collected for all applicants. To pay application fee using cash, check, or in person, please contact us to plan payment at one of our locations. Admission action will not be taken towards enrollment until the application fee is received.

3. Once the application form and payment have been received, an Allure Academy representative will contact you to discuss all interested programs of study options and provide you with the next steps and dates for starting this exciting educational journey.

4. In the next phase of enrollment, you will need to submit two (2) photos of yourself: The photos should be a recent head and shoulder shot (e.g., passport photo) and may be submitted electronically or in-person. (We are giving you the opportunity to gather your photos now while we process your application.)
